What Is Enhanced Care Management?

Enhanced Care Management (ECM) is a Medi-Cal benefit that provides one-on-one support to help people manage their health, housing, and life challenges. A dedicated care manager works alongside you to coordinate care, reduce barriers, and connect you to the right services at the right time.

Support understanding benefits, finding providers, and getting to the right care at the right time.

Individualized Care Plan

We build a member-centered plan and update it as needs change—medical, behavioral, and social.

Regular outreach to track progress, adjust plans, and reduce barriers as situations change.

Community Supports: Housing Services

If you have Medi-Cal and qualify for Community Supports, these housing services can help reduce barriers, stabilize your situation, and support your next steps toward longer-term housing. Availability depends on your health plan and individual needs.

Housing Transition Navigation

Support to help you search for housing, understand options, and plan next steps.

What this may include
  • Identifying housing options and eligibility requirements
  • Help with applications and documentation
  • Coordinating appointments and referrals
  • Creating short- and long-term housing plans

Housing Deposits

Help with certain one-time costs that can prevent someone from moving into housing.

What this may include
  • Security deposit assistance (when approved)
  • First month’s rent support (when approved)
  • Other approved move-in costs (when approved)

Deposit supports require authorization and depend on plan rules.

Housing Tenancy & Sustaining

Ongoing support to help people stay housed once placed.

What this may include
  • Help understanding lease expectations and tenant responsibilities
  • Support with routines, appointments, budgeting, and communication
  • Problem-solving and coordination when issues arise
  • Connections to services that support long-term stability

Transitional Rent

Transitional Rent is a CalAIM Community Support that may provide up to six months of rental assistance in interim or permanent housing settings for eligible Medi-Cal members who are experiencing or at risk of homelessness. Authorization and program rules depend on your Medi-Cal health plan. Transitional Rent cannot be used to pay back rent or prevent eviction.

A short-term bridge to help you stabilize housing and stay connected to care while you work toward longer-term housing. UWMC is coordinating implementation with Merced County Behavioral Health and Medi-Cal health plans.

What this may include
  • Up to six months of rental assistance in approved interim or permanent housing settings (when authorized by your plan)
  • Coverage of certain rent-related fees charged as part of rent (for example: some utilities, amenity fees, or storage fees), when allowed and authorized
  • Coordination with Enhanced Care Management (ECM) and housing supports to help you transition to long-term housing
Who this may be for
  • Medi-Cal members who are experiencing homelessness or at risk of homelessness
  • People with certain health needs (physical health, behavioral health, disability, or pregnancy/postpartum), based on plan criteria
  • People going through a major transition (for example: leaving a hospital, treatment program, jail, foster care, or interim housing), when eligible

Final eligibility and approval are determined by the member’s Medi-Cal health plan.

Medi-Cal Renewals

We help Medi-Cal members renew their coverage by completing paperwork, organizing documents, and submitting renewal forms. If you received a letter, renewal packet, or request for information, bring it in. We’ll help you respond correctly and on time.

No Cost

Renewal forms can be confusing. Our team can help you fill them out, gather what’s needed, and submit everything the county requests.

What to bring Any Medi-Cal letters, renewal packets, or document requests you received (mail or screenshots). If you’re not sure what you received, bring what you have. We’ll sort it out with you.
1) Review your notice We look at the letter with you and explain what it’s asking for in plain language.
2) Complete forms We help fill out renewal forms and organize required documents.
3) Submit correctly We help you submit paperwork online, by mail, or through other county-approved methods.
